ITS EMPLOYEE CARD SERVICE REQUEST
Requirement

The applicant is an ITS Permanent Employee.
-
The applicant submits a cover letter for loss of ID card from the work unit.
-
The applicant submits a certificate of loss of ID card from the police.
-
The applicant submits an official photo of good quality (selfie photos are not allowed) with a white background size of 4×6.

The applicant submits the name that will be included on the ID card a maximum of 25 (twenty–five) characters including spaces and symbols

The applicant prepares a file in the form of a soft file consisting of:

Cover letter for loss of ID card from work unit

Certificate of loss of ID card from the police

Official photo with good quality (selfie photo is not allowed) with white background size 4×6

The name that will be included on the ID card is a maximum of 25 (twenty–five) characters including spaces and symbols.

The PLT officer checked the file.

The applicant‘s recapture file is collected by PLT and sent to SDMO.

The process of printing ID cards.

The finished ID card is sent to the PLT for the access activation process.

ID cards are available for distribution and retrieval at PLT.
